This policy explains what information Restored Chiropractic & Wellness ("we", "us") collects through our website (restoredchiropractic.com), our practice hub and care portal (hub.restoredchiropractic.com), and our email and phone communications, how we use it, who we share it with, and the choices you have. It covers both our human patients and the owners of our animal patients.

What we collect
- Contact and account details: name, email address, phone number, home address, date of birth and sex (human patients and account holders), and a password you choose for the care portal.
- Patient information: for people, the health history, current concerns, medications, and other details you give us on intake forms, and the clinical notes we write about your care. For animals, the animal's name, species, breed, age, history, veterinarian, and the notes we write about their care.
- Visit information: appointment requests, dates, locations (including a barn, boarding facility, or home address for travel visits), and payment status.
- Payment details: we use Square to process payments and to store a card on file when you choose to save one. We never see or store your full card number; Square gives us only the last four digits, the card brand, and a secure reference.
- Communications: emails you send us and emails we send you, including appointment confirmations, reminders, receipts, and replies to your questions.
- Photo and video permission: whether you have said yes or no to us sharing photos or video of you or your animal.
- How you found us and similar practice-management details you choose to tell us.
- Technical information: the basics needed to run a secure website, such as the type of device and browser you use and the time of your visit. We do not use advertising trackers.
How we use it
- To schedule, provide, and follow up on care for you or your animal.
- To send you confirmations, reminders, receipts, and replies you have asked for.
- To keep accurate records as required of a healthcare practice.
- To bill and collect payment.
- To let you sign in to your care portal, book visits, and manage your information.
- To run the practice, for example understanding how many visits we see by month or which services people use, using counts and totals rather than individual details.
- With your separate permission, to share photos or video on social media.
We do not sell your information, and we do not share it with advertisers.
Who we share it with
We share information only as needed to run the practice, and only with providers who agree to protect it:
- Hosting and database: our hub and portal run on Vercel and store data with Supabase, using industry-standard security with encryption in transit and at rest. Clinical notes are additionally encrypted with a key that only we control before they are stored.
- Payments: Square, for invoices, card-on-file, and payment processing.
- Email: our email provider, to send and receive practice email.
- Maps: Google Maps, to estimate drive time and distance to a travel visit address. Only the address is sent, never your name.
- AI assistance for two limited, staff-only tasks: we use a third-party AI service (Anthropic) to help draft a reply when you email us, and to help organize uploaded documents by reading a chart file so it can be filed under the correct patient. Nothing is sent to that service automatically. A member of our staff chooses to use it for a specific email or document, only the information needed for that task is sent, it is used solely to produce that draft or suggestion, and it is not used to train AI. Every draft is reviewed by Dr. Patrick before anything is sent to you. We do not use AI to make clinical decisions or to send you anything on its own.
- People you authorize: other medical providers, family members, or others you have listed on your intake or in a written release.
- When the law requires it: for example a court order, or a report we are legally required to make.
Family accounts and the care portal
At your request we can connect more than one login to one family file so, for example, spouses can each sign in and book visits for the family and its animals. Everyone signed in to a shared family file can see the family's upcoming visit dates and times. Clinical notes about a person are never shown in the portal to anyone unless we specifically share that note, and we only share an adult's note where the people on the account are covered by that person's release of information. A parent or legal guardian may see a minor child's notes that we share. Tell us on your intake, or any time, if you do not want family members to see your visit dates, and we will keep your file separate.
How we protect it
- All connections to our website, hub, and portal are encrypted (HTTPS).
- Data is encrypted at rest by our database provider, and clinical notes are encrypted again with our own key before storage, so they are unreadable without it.
- Staff access requires a password and two-factor authentication. Every time a clinical note is viewed, edited, or shared, that action is recorded.
- You control your own portal login. If you forget your password you can reset it by email; if you change your email you must confirm the new address with a code before it is used for sign-in.
- Files you upload or that we attach to your file are stored privately and are only ever shown to you through short-lived, signed links.
How long we keep it
We keep health records for as long as North Carolina law and professional standards require, generally at least seven years after your last visit, and for minors at least until age 21 or seven years after the last visit, whichever is later. Account and contact details are kept while you have a relationship with the practice. If you ask us to close your portal account, we remove your login while keeping the underlying health record for the required period.
Your choices and rights
- You can see and update your contact details, address, birthday, and communication preferences in your care portal at any time.
- You can ask for a copy of your records, ask us to correct something, or ask us to restrict how we share your information. See the Notice of Privacy Practices for how.
- You can change your answer about photos and video at any time.
- You can opt out of reminder emails and marketing emails in your portal or by telling us. We will still send messages needed for your care, such as a confirmation of a visit you booked.
- You can ask us to close your portal login at any time by contacting us.
Children
Our care portal is for adults. A parent or legal guardian creates the account, adds a child as a patient, provides the child's information, and signs the consent on the child's behalf. We do not knowingly let anyone under 18 create an account.
Changes to this policy
If we make a meaningful change we will update the date at the top of this page and, where the change affects how we handle your health information, we will let you know at your next visit or by email.
