Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

This Notice applies to our human patients. It describes the practices of Restored Chiropractic & Wellness and Dr. Patrick Rebadow, DC. We are committed to protecting the privacy of your health information, and we follow the standards of the Health Insurance Portability and Accountability Act (HIPAA) as a matter of practice, whether or not a particular rule technically applies to us. Information about our animal patients and their owners is covered by our Privacy Policy.

Our duties

We are required to keep your health information private, to give you this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect. We reserve the right to change our practices and this Notice, and to make the new Notice apply to information we already hold. If we do, the updated Notice will be posted at hub.restoredchiropractic.com/notice-of-privacy-practices and available in our office.

How we may use and disclose your health information

For treatment

We use your health information to provide, coordinate, and manage your care. For example, we may share information with another provider you have asked us to work with, such as your physician, physical therapist, or a specialist you list on your intake or in a written release.

For payment

We use your information to bill you and to collect payment, and to give you a receipt or superbill that you may choose to submit to your insurer or health savings account. We do not bill insurance directly.

For health care operations

We use your information to run the practice: keeping records, scheduling, sending you appointment reminders and confirmations, reviewing the quality of care, and administrative tasks. Where staff use a computer tool to help draft a reply to your email or to organize an uploaded document into the correct file, only the information needed for that task is used, a member of our staff reviews everything before it reaches you, and no such tool makes clinical decisions.

Appointment reminders and follow-up

We may contact you by email, phone, or text (if you have given us a mobile number and agreed) to remind you of a visit, confirm a request, or check on how you are doing.

Family members and others involved in your care

With your permission, we may share information with a family member, close friend, or other person you identify as involved in your care or payment. You can name these people on your intake form and change the list at any time. If you have connected more than one login to your family's care portal account, the people signed in to that account can see visit dates and times; we do not show them your clinical notes unless you have authorized it.

Minors

A parent or legal guardian generally acts on behalf of a patient under 18, signs consent for care, and may access the child's health information, except where North Carolina law gives the minor the right to consent alone.

As required or permitted by law

We may use or disclose information when required by law, for public health activities, to report suspected abuse or neglect, in response to a court order or lawful subpoena, to avert a serious threat to health or safety, or as otherwise permitted by law.

Uses that require your written authorization

Any use not described above requires your written authorization, which you may revoke in writing at any time (except to the extent we have already acted on it). This includes marketing, any sale of information (which we do not do), and sharing photos or video of you, which we only do with your separate written permission.

Your rights

• Access. You may see and get a copy of your health record, in paper or electronic form. We may charge a reasonable, cost-based fee for copies.
• Amendment. If you believe something in your record is wrong or incomplete, you may ask us in writing to amend it. We may deny the request in certain cases, and will tell you why in writing.
• Accounting of disclosures. You may ask for a list of certain disclosures we have made of your information, other than for treatment, payment, and operations, for up to six years before your request.
• Restrictions. You may ask us to limit how we use or share your information, including with family members. We are not required to agree to every restriction, but we will honor a request not to tell your health plan about a service you paid for out of pocket in full.
• Confidential communications. You may ask us to contact you in a specific way or at a specific place, for example email only, or a different phone number, and we will accommodate reasonable requests.
• Paper copy of this Notice. You may ask for a paper copy at any time, even if you agreed to receive it electronically.
• Breach notification. If your unsecured health information is involved in a breach, we will notify you as required by law.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us at the contact information below, or with the Secretary of the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
